In April, the Afghan president, Ashraf Ghani, granted Nakamura honorary citizenship of Afghanistan. Since 1984, he had worked in the region and initially providing aid in Peshawar, in north-west Pakistan, before opening a clinic in Afghanistan in 1991.

Locals credited Nakamura with transforming vast areas of the desert into fertile farming land. He said on a Japanese public broadcast show that he found inspiration in the teachings of ancient Buddhist philosophy. As of 2016, Nakamura had built or restored 8 additional canals, irrigating 16.000 hectares and supporting the livelihood of 600.000 people in the Gamberi Desert region.
